Elko Co, NV (October 01, 2021) – Troopers from the Nevada Highway Patrol are investigating a fatal multi-vehicle accident that took place on Monday, September 27th in northeast Nevada.
The deceased, identified as 70-year-old Darryl Mayhew, a resident of Troy, IL. was involved in a chain reaction collision in a construction zone on US-93 between Jackpot and Wells at about 7:40 a.m.
A source from inside the department said that a Penske moving truck traveling northbound on US-93 approached traffic stopped by a flagger in a construction zone. The Penske operator failed to stop and crashed into a Chrysler Pacifica van. The impact pushed the Chrysler into the Freightliner dump truck in front of it, and then onto a dirt shoulder.
The Penske truck twisted and overturned, striking the dump truck, which then collided with the rear-end of a Ford F-150 which, in turn, hit the back of a Dodge van.
Mayhew, the driver of the Chrysler, died at the scene. His passenger was med flighted to a hospital in Utah in critical condition.
The occupants of the dump truck and the pick-up sustained minor injuries and were treated at the scene.
The NHP is investigating impairment and inattention by the Penske driver as possible causes of the crash.
